- Complete the Patient History Form.
- Complete the Consent Form.
- Complete the Pain Assessment Form
- Complete the Patient Anxiety Scale.
- Complete the HIPPA Disclosure Form.
Once these forms are completed you can now call Elevated Health and a member of our team will help you schedule your consultation and screening.

You will also learn more about the treatment protocol, what to expect during the infusion process, and ideal outcomes. Most steps in this process can be completed in person at your clinic of choice or remotely via phone or a secured web conferencing system.
Each consultation is done in our clinic by Dr. Darin Stettler, our Board Certified Anesthesiologist, who will evaluate you personally and make a recommendation for your care.
There is a consultation fee of 100$. That fee will be applied to your first infusion if you choose to continue with treatment.
In most instances once treatment has been approved an infusion can be done that day. Most treatment plans are broken down into two parts. 1-Stabilization period which typically lasts two weeks, with two infusions per week. 2- Ongoing Maintenance period which varies from patient to patient.
